October 15, 2018: All About Me Tool | Evaluation | Appropriate Use of Antipsychotics Webinar

Hello Clear teams!

Thank you for attending the Appropriate use of Antipsychotics Webinar last week. We had an interactive, engaging discussion around the use of antipsychotics and learned about different approaches from a family/care partner perspective. A special thank you to Johanna Trimble for highlighting the importance of deprescribing antipsychotics and to Susan Conklin for sharing such a touching story about her mother Gwen’s journey and the impact deprescribing had on both of their lives.

Resources

At a Long Term Care home, knowing more about the people you take care of helps community support workers and staff build relationships and support personalized care. “All About Me” is a resource for people with dementia to tell health-care providers about themselves –their needs, likes, dislikes and interests.

 Webinar Recording

While it’s not the same as being there, if you missed the session or want to share your learning with your Clear team, access the recording and other materials here. The materials – presentation slides and the resources/tools discussed are also attached for your convenience!

‘Sharing the Goods!’ // Kudos

Since Clear started, Jerry Joseph and the team at Simon Fraser Lodge have been working hard to bring everyone along the journey to reducing and eliminating anti-psychotics for their residents. Lots of small changes have been put into action such as meetings that include pharmacists and allied health to looking to sustain achievements through more education for team members. The result? Since August, there has been a 32% reduction in anti-psychotics for residents!
