In 2003, Fraser Health committed to an initiative called “Let’s Talk” to address the advance care planning needs of their patients and families. The title “Let’s Talk” was chosen to accentuate that dialogue about health care choices which facilitate individuals to think about their values, beliefs and wishes must be inclusive of all parties – patients, loved ones and health care professionals.
Over the last seven years, the FH Advance Care Planning Team has championed the development, implementation, evaluation and dissemination of FH’s Advance Care Planning program. Key components developed include Advance Care Planning educational on line modules and workshops for healthcare professionals, public pamphlets and posters, the My Voice workbook© and systems to support the implementation of advance care planning throughout the health authority.
