Cheryl Mitchell is a freelance facilitator and advisor in the health care industry and public service who has provided organizational systems solutions for more than 20 years. She holds a PhD in Human and Organizational Systems, where her dissertation research focused on blame in the health care system.

During her plenary presentation at Quality Forum 2016, Cheryl told us about what it means to foster a culture that is blame-free and idea-rich and fosters innovation.
