Project description:
Patient and staff incidents happen daily in health care. Incidents are rarely debriefed to find out what happened and what could be done to prevent it from happening again unless someone is severely injured.
An operational debrief focusses on reviewing and identifying areas requiring attention and/or changes.
Debriefing can help prevent future unsafe incidents and improve patient care and staff safety e.g. procedural gaps, potential patient triggers and safety interventions.
Majority of staff and leadership lack the capacity and tools to consistently and formally debrief patient and staff incidents.
The project goal is to increase the number of debriefs facilitated by 4W staff per month on an inpatient medical unit by 50% by the end of December 2018.
