Transition Services Team (Acute-to-Community) Standard Work

Project description:

The Vancouver Coastal Health Authority (Vancouver Community & Acute) has undergone significant changes in the delivery of care for patients discharged from hospital requiring services in the Community. Regional initiatives & models of care such as “Home is Best”, “Ideal Transitions Home”, and the use of the iCARE methodology within acute care have been implemented since the Transition Services team (TST) was formed. It is time to revisit the TST work in light of these new initiatives & directions to develop & implement practice standards that will ensure that each patient in hospital will receive the services they need upon discharge to home/community without delay. The scope of the project includes:

  1. Design & Development – development of new practice standards and the standard work & orientation strategy/structure needed to support the new standards
  2. Implementation – putting the new standards & orientation structure into place for both new & existing staff
  3. Sustainability – inclusion of practice support strategies for each standard to ensure that the newly-developed practice standards are being incorporated into the day-to-day work of the TST.

The new standards will apply to TST work at all Vancouver Acute (VA) sites (VGH, UBCH & GFS).
