Medication errors have a substantial impact on patient outcomes and system resources (medication reconciliation is beneficial and cost effective). A complete and accurate medication history upon admission to the hospital is an integral part of patient assessment. In addition, reconciling medications at the time of discharge from the hospital is critical to ensuring patient safety, preventing unnecessary re-hospitalizations, and reducing the risk for medication misadventures.
The Northern Health PharmaNet (Medinet) Pilot project, located in the Northeast Health Service Delivery Area (NE) of Northern Health (NH) has shared their success with the rest of NH as well as other medication reconciliation teams across the province.
